What does a team-based (as opposed to a hierarchical) organizational chart look like?

Do you mean a HPO (High performace organization)? Teams are usually small and have one group leader to keep the discussion going and make sure everyone is on task but when it comes to it everyone has a single vote. So there isn't really much of an organizational chart. Hierarchical organizational charts look almost like a tree and it shows who reports to whom and that gets all the way to the top guy who usually reports to a team such as the board of directors. At least in a traditional corporation.
