When my cats get that "deer in the headlights" look, like they're going to throw up, I grab them and put them in the kitchen, so I'm not cleaning up the floor. I think, when it happens, it's pretty quick for them, and they don't have the mind capacity to run to the litter box. I'd be happy if all of mine would just use the litter box for pooping, instead of my floor.

Because the urge to vomit is too quick for them. They don't even think about going to the litter box, as many cats will even eat it back up, something they don't do with their poops. And the urge to use the litter box comes on with enough warning that they can find a safe place to do it, they use thought to go to the bathroom, not just going anywhere, they choose where to go.

It's not like they have any warning when it's coming. They don't suddenly think "OH my, I think I'm going to puke in a few minutes, I best be finding an appropriate place to do so." and go off to find their puking place. They're just minding their own kitty business, thinking "la la la, I'm a cat, I'm laying in the sun, la la la, Im a... HURK HURK HURK <insert more vomiting noises here>...." Then it's just done and over with. I do have to say, though, that my cat "pukes" once a month or less. It's not often enough to worry about where she's doing it. If your cat seems to puke often, perhaps it's best to check with the vet to see if there's an underlying problem. Or, if it's caused by hairballs, ask them to suggest a hairball prevention method that would work well for your cat. They sell hairball goop, and treats with hairball goop in them, to help the hair pass through the digestive system instead of being regurgitated.

Why on earth would a cat want/need to puke in the litterbox?

It's not hard, it's hard 2teach'em how to hold it in 'till it's time for it to go outside!! *see photo

Cats usually don't have enough "warning" when they are going to be sick. I don't know of anyone who has a cat that barfs in the litter box.
