The easiest way would be for the person who you are transferring it to to create an account at the same registrar where you have your domain, then move the domain from your account into his/hers. Every domain registrar has a slightly different procedure to do so, but it's usually pretty straightforward. GoDaddy calls it "Account Change", eNom calls it "Pushing a domain to another account", etc.
