I think NYC is about a half dozen counties. Hospitals rarely keep readily available birth or death records. They always give those to Vital Statistics, and only they can sell you a notorized copy of the original with the embossed seal. And that is what you need for a passport or other similar use. If you still haven't narrowed te county, your New York State Department of Vital Statistics hold a transcript that will steer you to the correct county.
