There are about 100 different forms of RA, and in juveniles it acts differently. My daughter was just diagnosed and it's a struggle to get a grip on it. There is currently no known cause, but they are doing studies regarding genetics. RA in adults may cause hot, painful swelling, but in kids only one of the forms causes the joints to feel warm. Usually a fever and a rash go along with it. Other forms aren't painful and goes for awhile undiagnosed until the swelling of the joints is really obvious. It's a very complicated disease. It can also cause uveitis which is an inflammation of the eye and many other auto-immune issues such as lupus. It is definitely a complicated disease. But the body is basically attacking itself and destroying the joints. You can see in the pic how swollen the knuckles are.
