Could a big heap of underwear on the road stop a car or will the car just drive right through them? Would the underwear somehow get entangled n the wheels or the moving parts of the car e.g. fan/shaft?

It would have to be a very big heap. I would say a car would probably turn over first after a jump on such a heap. Stopping a fan won't stop the car, and your underwear has to be bulletproof to even try stopping the crankshaft at full speed.

Doubtful there would be any damage, although if it is big enough you may dent or damage your oil pan.
