Unscrew it, and then take it out?

Just as stated, unscrew it and slowly, carefully take out. I may help to do this in the shower, wash it and then take it out. That way it will be clean and the skin will be soft making catching and tearing less likely.

If it doesn't want to unscrew, try wearing latex gloves for a better grip. Of course, don't do this if it isn't healed yet, especially if it's externally threaded.

Id be more than happy to help you out ;-) I kid! Best time is after a shower. And like someone else said use gloves for a better grip. If you still cant do it go back to the shop you had it done at and they can change it for you. Thats kinda the best way with a piercing like that, when any ripping would not be too much fun.
