What do you do when you know someone is speaking english, but you can't understand them?

Go to sleep. I'm either very tired and people start to sound like the adults in the Peanuts cartoons, or I'm on something and should probably sleep it off.

Automatically assume I am in Kentucky. I can't understand my relatives from KY at all.

I repeat what I heard in my head phonetically until I figure it out.

English has soooo many dilects and sometimes is frustrating, I just do the Polite thing and where they are from and inform them that there vowels and words are different from mine and are difficult to undrstand and then ask then to slow down a little.
