Weight is determined the following way: Empty weight of the aircraft plus furnishings and crew is the where we begin since it is a number that is already known. This weight plus fuel and passengers/cargo becomes the BOW (basic operating weight). Each passenger and each bag adds more weight, and is therefore added with a generic equation, each person + carry on is another 190 lbs. Each piece of luggage added is 30 pounds, with seasonal adjustments made for heavy clothing. It is also important to note that this becomes the gross weight of the aircraft on the tarmac. Further calculations are performed to account for fuel consumed by the APU until start up and also to accomodate for fuel consumed during start up and taxi. This weight is the responsibility of the captain and the first officer, who both perform multiple calculations in the cockpit prior to takeoff. Sometimes aircraft are physically weighed beforehand by rolling over scales before departure as a safety check and a means to get the actual weight.
