Why is it that some people comment with 'thank you' or 'good answer' but do not rate it accordingly?

They rate it accordingly to how they see fit. It could still be a good answer but not a great answer.

I think sometimes people just plain forget. It's not possible to identify anyone who rates you either up or down from looking at the ratings feedback. Sometimes people will tell you that they've uprated or downrated you.

Sometimes they forget some new ones don't know they can just go with the flow!
