First off both players have to drop their sticks, their gloves, and be willing to fight. Lots of fights that get broken up start as scrums in front of the goal crease because the opposing team is trying to protect their goalie. So, it's a bit unorganized at that point. Sometimes one player will have his gloves on and the other will have his gloves off or two players will be holding on to one. A lot of the time it's to prevent something bigger from happening. The refs want a clean one on one fight, not an absolute hockey riot. Although those can be entertaining (Go to youtube and search for Buffalo Sabres vs Ottawa Senators hockey fight) I imagine they are an absolute mess for the refs to sort out.

To add to that, the fight can't go to the floor.
