How are short term memories changed into long term memories? And how long does it take?

I can't answer on how long it takes, as I think that it differs from person to person, but I do know that it involves repetition of the act, and active use of it. eg I speak 4 languages. I used them constantly, so I am pretty good at them. But I learnt calculus 30 years ago, passed my exams and then never used it again. Now I cannot remember even the basics.
