An accident could happen, or you could do something crazy, like run into a fire trying to blow it out -- but these things are pretty rare -- I never hear about them -- like a "worst case scenario." This is what the FDA has to say about it http://www.fas.org/irp/agency/doj/dea/product/lsd/lsd-7.htm Taken in the proper dosage, LSD is as safe as any other pharamceutical... taken in the wrong dosage, LSD is as dangerous as most pharmaceuticals are.

It causes hallucinations. I'd say yeah.

LSD will not cause death. If you make yourself paranoid and start freaking out you may elevate your heart rate this could cause problems, no worse than being very neurotic and being stuck in traffic.
