If you are in the UK, it can be done same day, but you need to travel to your local passport office, http://www.ukpi.org has details.

In the USA, it depends on what time of the year you apply. I got mine in Ocotber in about 30 days. That is considered pretty fast. Spring time is busier, so it may take longer then, maybe 60 days. You can also pay extra to get it expedited. The following is required when you request a passport: 2 photos for passport application (you can get these done cheap at places like Wal-mart or Costco), current license or ID, proof of citizenship, Home telephone number, Date of trip, countries to be visited, Emergency contact. Also keep in mind that some nations require a visa (China for example). So after you have a passport, you may need to get approved for the visa as well.

New Answer for The United States :-) 2 weeks is the best I think, see http://travel.state.gov/passport/get/processing/processing_1740.html

I think the "normal time" is 4-6 weeks - but there is a new regulation due to go into effect (at any minute now) requiring passports for US travel to Mexico and Canada. As a result, there is a HUGE demand for passport processing and the Homeland Security Dept did not ramp up staffing to manage the demand. As a result, processing time is currently taking 15 weeks or more. There was a news article dated June 7, 2006 indicating the government was in HUGE backlog for passport requests and will not be enforcing the passport requirement for travel to Mexico and Canada quite yet. The article doesn't indicate how much longer they will waive the new requirement. Check with the state department website. There are services that will "walk" your passport application through the state department on your behalf, but they are expensive. in the mid 80's the service was over $100. I would imagine the price has increased considerably since then - particularly given the current backlog in application processing.
