Hmm...good question. I would say that whether it was intended as a compliment or an insult would depend on the tone and context. But whatever it's intention, it lands as an insult.

I'd say it is just a crude statement, and I would not take it as a compliment at all. If you want to compliment someone on how they smell, I don't think there is any reason to even have the word douche in the statement.

Unused? That means fresh, flowery, but slightly on the vinegary side. I would take it as a compliment and that they are idiots!

Well, it implies a fresh smell but I would think any comparison to a douche would be taken as an insult.
