Generally speaking black tends to mean necrosis. This is very common in diabetics due to peripheral neuropathy. The fact that these spots are growing is not a good sign. He needs to go to the doctor.

If the "black spot" grows out and disappears, it's a sign that he's injuring the nail and it bleeds underneath. If it's happening again, he should go see his nearest podiatrist to get it checked out. He might be wearing too small of a shoe and keeps injuring it. If the black spot stays in place but gets bigger, that's a sign that it could be something else (i.e. melanoma) which needs to be biopsied right away. Good luck!

Hi, My name is Miranda and i'm having the same problem too. Under both of the big toenails, I have spots. It doesn't hurt at all but, i'm really scard it might be somtihng serouse!!
