Not only does lettuce have carbs but the other ingredients do as well. Lettuce has some carbs probably under 1 gram. Cheese is under 1 gram. Onion and tomatoes have carbs as well.

By leaving the Ketchup off the burger you bring the carb count to 2 net carbs. The ketchup is where you get most of the carbs.

Hardees has a low carb thickburger served in a big lettuce wrap. They also have a traditional thickburger served on buns. Unless you ask, by the way, even the low carb version comes with ketchup, which is full of sugar.

Ask to have the mustard and mayo removed as well as these places buy these condiments in bulk, which means sugar is added as a preservative. I buy Duke's mayonaisse, which has no sugar. I bring the burger home and add the mayo myself ;) Thankfully, someone gave the correct answer that the ketchup is where most of the carbs in the burger are coming from.
