My boyfriend Is kinda willing but worryed my hubby will change personalities midway through I am willing but same fear

- I am DID and so is my same sex partner. Cheating is out of the question. There is already too much insecurity and to add to it would be dangerous, especially for me. All of me loves all of her and vice versa. just sayin that your behavior is probably contributing to the problem and compounding his paranoia. You are not a nice person. Have some respect.

Sounds like a recipe for disaster to me. There isn't just the person you refer to as "your hubby" to consider but all the people that make up who he is inside of him. All those personalities are still holding each others hands and heart. Show some respect for all of who he is.
Perhaps his other personalities don't want their body doing a threesome.
You already know he can change midway or you wouldn't have mentioned it.
You really should get some counseling about your husbands condition.

- It's to each their own. Some people dislike monogamous relationships and would prefer to be close with more than one person. That's their decision. Just because it doesn't suit your moral sensibilities it doesn't mean it's wrong, and it certainly doesn't mean this person is cheating on their husband. He's obviously aware of the situation.
  
  If you're offended by it, don't respond. Don't stand there on your soap box and suggest people are morally corrupt for not living exactly how you do.

- It's highly unlikely that an extra marital relationship is to blame for his mental illness. There is no evidence to even suggest it could be a cause.

- You can't really know that without knowing the case history. If it contributes, it would be evident. Since we have no case history/evidence of such, there's no point speculating that it adds to it.
  A lot of people can suffer mental illness and not be affected by such things, particularly if that's their stand point in the first place. You can't know without seeing the history.

- I wondered that as well. Maybe it means "Dissociative Identity Disorder". She does say something about her boyfriend and her being worried about a personality change, so that seems to fit the context.

I would discuss it at length with your husband, and raise your fears about what might happen. Raise the same issues with your boyfriend as well. Don't just jump head first into it, make sure everyone understands what's going on, and each others worries and expectations.

If you have any doubt over whether it would be a good idea, or any fears over your husbands personality changing, then I would suggest not doing it. You shouldn't go into something like that if you're afraid of something.
