Are there people (adults) who truly don`t understand the consequences of their actions? I know someone who does stupid things and then seems surprised that there are serious consequences. Is there anything that could prevent this from being learned?

Several things but I would put just not thinking at the top of the list -some, many, most are on auto pilot and are reactive, not proactive. Lazy may be a close second - why worry or even care seems to be the line of logic of some, if things go too far awry one can always complain about how unfair life is. +5

I think soo my daughters ex,i think it was the way he was raised.his mother taught him nothing was his fault everyone else was always too blame.
