I am sorry, please forgive me for any perceived rudeness. The term "apologize" is meant to give a reason for the action. It's historical definition is: a defense, excuse, or justification in speech or writing, as for a cause or doctrine.

Always and immediately. When I made a mistake raising my son I immediately apologized and he learned early on that parents are human and make mistakes just like children do. If I made a mistake at work I would always immediately tell my supervisor and not try to hide it or fix it without his/her being aware of it. I can't live any other way. :)
Yes, especially if you do something really mean. Otherwise you could create a suicide.
The only mistakes I ever make ARE apologizing

Yes, some are too quick to apologize or apologize even though they didn't do wrong. I work with someone like that :-)

LOL I have learned not to. I say I am sorry when it is appropriate. There are those who refuse to admit fault. I will not apologize to them.
