There are two things that make it work. One is the input from everyone. This can be done on the website or through an API provided by Twitter. The second is output to all your friends and people's timelines. These are typically individual queries that can be done through the website or an API. The biggest problem with Twitter is the way to scale that many users. That is why it has lots of problems. Lots of new technologies are put in to handle thousands of messages a second all day everyday.

You sign up for an account and 'follow' people, which means you can see their updates (Tweets) if their tweets are public. If they're private you can only see them if they follow you too. People send updates about whatever, in 140-character chunks. It gets more complicated than that but that's enough to get started. :)
