First after completing Act 4 ( or act 5 for expansion pack) Be in the same difficulty you just completed, go back to Tristom in Act 1, pick up Wirt's leg and go back to Act 1 camp, put leg in tome of town portal in Cube and click transmute then the portal opens to the Cow level.
