assess the damage first and go from there, if it's just chipped paint or something then ignore it, it's much too minor, but yes you should leave atleast a phone number regardless.

If there was no damage done, at all, then I would probably just leave the scene. But if there was the slightest bit of damage then I would probably leave my name and number

I'd stop and write a note just in case someone was watching. it would read Dear car owner, I hit you car. Its damaged pretty bad. I am only writing this note to give the illusion i am giving you my information, just in case someone saw. Best luck with the repairs!
