first, try putting gloves or something over his hands so that he can't use his nails to scratch the area. If it still does not go away, or still seems to be spreading, visit the doctor. It could be something more serious. Sometimes, if a toddler scratches himself by accident and it starts to heal, it will itch and then he'll start to scratch in his sleep. This could cause redness that could resemble a rash. The kid could also have sensitive skin, to things such as metals, or certain lotion types. So if visiting the doctors, see if there is a certain test they can perform to check for these things. In adults, these things could include belt buckles, lotions and deodorants, so I'm sure there could be similar things effecting your toddler. I hope this helps.
