Are language courses on the Internet any good? Has anyone ever learned to speak a new language fluently by using this method?

There is no doubt that there are some good language packages out there. I suppose the same question could be asked if people have ever learned a language by merely reading books? I have used a book to learn a language, but I was able to back it up by talking to real people, and improving my language that way. As I see it, there is no substitute for talking to people, however, there is no reason why using a good grammar based book cannot helo you with the basics. Replace the good grammar book with an internet course, and I cannot see why it cannot help.
