Having candles burning and leaving them unattended. I did that once...candle in living room on end table..went into the kitchen for a few minutes. When I came back something seemed different..the candle was gone..I looked on the ground and to my horror the entire bottom of the sofa was ablaze. The candle had come out of the holder somehow, dropped on the floor and rolled under the sofa. I never did that again. If we have a candle lit, someone is always in the room with it or we put it out! :)
