Emergency numbers are different all around the world. 911 is only an emergency number in the U.S./Canada/El Salvador and parts of Brazil. Everywhere else it is different. Cell phones sold in the U.S. and Canada are required by law to be able to dial 911 even without a SIM card, however a phone dialing 911 in Germany wouldn't call anyone as their emergency number is 112. In the US you will reach the local 911. They can tell from which tower you're using. Some cell services allow emergency lines to even triangulate your position with incredible accuracy. Outside the US you would have many more problems as already explained.
