God! What an intelligent question! Being an ex-addict, I would have to say that at first it is a choice to "use', but then it becomes an addiction that is uncontrollable to the point of a disaease

Addiction is not a disease; it's often a choice and always a weakness. And, yes, the Catholic church and their screwed up AA program were the ones that started condoning & justifying alcoholism by calling it a disease

In the USA health care system, the term "disease" has morphed to include numerous things that would never have been considered a disease before. But usually in order for medical insurance to pay for something, it needs to be classified as a disease.
