Ask. both ways, just ask ask. Hopefully the girl isn't stupid, and realizes that being honest is going to make it better for her. You'd hope that the moans/groans/thrashing/etc would make it obvious, but so many girls fake now that it's easy to think someone is faking. The best signs, as crude as this may sound, is how wet she is, and how much she initiates sex. Sex itself, I personally find very limiting. But if you alternate with oral, or do oral then sex, it can add more. Most women aren't able to orgasm during sex (or only in specific positions/times, each woman is different) and oral is the -best- way to give her climax, which I'll define as "fun". But talking to her about it is probably the best way. Come up with questions that aren't direct, but are fun, if it's difficult to talk about. Like "what's your favorite position?" - and make it into a game, and see where it goes. Asking can go a long way, though.
