The Microsoft ASP.Net 2.0 has a membership framework which contains all the bits of this built-in. It's freely available for download, but of course the skills to use it don't come with the file! I'm sure there are open-source frameworks for this too, but I'm less familiar with those.

I would also add that you can find the snippets of code for this and many other functions at koders.com It's a site that I use almost daily to prevent myself from trying to reinvent the wheel.
