We get a milkshake and chocolate and leave with the satisfaction of doing something good for another person. Although, that said, people don't pay for blood transfusions either.   • To respond to those from outside the U.S., Americans can simply donate blood to the Red Cross, but you can also sell plasma. Blood is taken from your arm, the plasma is taken out, and then blood is put back in. I hear that the plasma is used for testing things like cosmetics. So, decidedly less noble, but a way to make money. Do you not have plasma centers in the U.K. or Australia?
