While I'm sure many will suggest "The Fight Club" has the best plot twist, I'm going to go with the end of the 1987 Kevin Costner film "No Way Out." That was awesome.

I would have to say the movie "Wild Things" with Kevin Bacon and Denise Richards. It had more twists than a bag of pretzels!

Pulp Fiction. I didn't understand that movie the first time I saw it until the VERY END! And Sixth Sense. Everything just settled into place at the end in a way that was TOTALLY unexpected for me.

Personally, I think The Game. The first time I saw it, the ending just blew me away. I only saw it because I watched part of it being filmed in SF and was curious. I am SO glad I saw it too. It still sticks with me. Anybody who mentions M. Knight Shamalamadingdong has no idea about quality movie making. Sure, his twists are unexpected.... like the weaving of a drunk about to puke on your shoes, and about as entertaining.

Haute Tension aka Switchblade Romance or High Tension depending on what country you are in....alot of people didn't like the ending but I enjoyed it. Good horror movie if you haven't seen it. : )

The Devil's Chair. I don't know how much it counts as an actually plot twist...you would have to see it to understand...but it's very original, scary and kinda funny, too. "IS THAT WHAT YOU CAME HERE TO SEE, YOU GORE HUNGRY MIDNIGHT FREAKS?!" Instant classic. Jaccob's Ladder is still the best plot twist ever though. While using a complex and psychologically numbing premise, they made it even more weird with the actual significant plot twists, it all makes sense in the end, is very original, and for something so MESSED UP, they did a very good job to keep it making sense.

The Sting by far...every time I see the movie I understand more of what was really taking place...You cannot watch the movie only once or twice because there are so many different things taking place from the first scene even before the credits roll to after the End comes up. Brilliant movie and music also

Pitch Black (when the blonde chick, the main character, dies) really threw me for a loop, and not many movies do that.

I was surprised by the twist in The Matrix and also surprised by The Sixth Sense (that is, unfortunately, the LAST time I've been surprised by an M. Knight Shyamalan movie. He's become predictable and formulaic.
