Sometimes. I thinks its just a polite gesture sometimes. If you think about it, we ask everyone how they are....our parents, kids, people in the checkout line at the grocery store, the car wash people...it's just a common phrase we say because what else could you say....

It depends on who I'm talking to. When I'm at work and I ask how they're doing, I don't really care. If it's a friend or family member, yeah, I care.

Honestly? It depends on who I'm asking. Sometimes I'm just asking to be nice or make conversation...in which case, there's a pretty good chance I really don't give a damn. I'm kind of mean like that;) But other times I'm asking someone who matters to me on a personal level. In which case, I actually do want to know.

Yes, because I would not greet someone I don't care for with those words.

In some instances it is a form of greeting for me, but that changes instantly if I see they want to talk about something.

Yes, that's why I'm asking. I don't do it just to be polite or to pretend I care. I do care. And if it's someone I don't know and I don't care about their health for some reason, I'll talk about something I do care about. I'm not into small talk.

Generally. I try not to slip in a "how's it going" or "how ARE you?" into a polite greeting if I don't really want to know (I'll say "hey, good to see you," or "hope you've been well" instead), but sometimes it slips! Usually if I'm asking how you are, though, I care and would like to hear about it. I'm not a "Hi how are you?" "Fine thanks" sort of conversationalist. I don't like fake small talk :)

I do. Most people don't. It has evolved into a common greeting rather than an actual inquisition of ones state.

When I ask at work! I truely most of the time do not care but I play it off well! On here though or in my personal life if I ask then I truely mean it! :)

I never ask. Simply because, really, I don't care.

When I'm on the clock. Not really. When I'm talking to people I really care about. Yeah.
