My personal opinion is that they suck, regardless of the biological mechanisms in place.

technically they do both. First they bite a hole into you and release an anti-coagulant in you and then stick something in they use to suck your blood. And it's not the bite that hurts, it's the anti-coagulant that stings so bad.

Aside from human females Mosquitoes are the only known creatures that can spit and suck at the same time.
