I think the most difficult thing people go through is fear, whether it is stemmed from or a symptom of pain, loss, frustration, the unknown, or ignorance. I see so many people have such a difficult time dealing with fear...and some don't even know that what they are feeling is fear and they often mislabel their emotions/feelings/thoughts. Overcoming fear follows growth.

losing your mother or child has got to be one of the hardest life changing things ever

Slowly watching your child die, and knowing that nothing and no one can do anything to help. That's hell.

Being heartsick. I imagine that everyone can relate to that and knows how miserable it is, no matter who they are.

Probably like others said, dealing with mortality.

I think that it would be betrayal of someone you thought you trusted. This would hurt you emotionally and will never be out of your mind ever. This will also lead to not trusting people hardly at all anymore and that can detrimental to future relationships you have with people.

I think the death of a child would be the most difficult thing imaginable. Second to that, I would have to say the loss of one's parents. That is certainly the very hardest thing I have ever dealt with.
