The idea of it was that it was seemingly plausible that the fantasy world was real, but at the same time also believable that she was just imagining it. For example, she could have gotten muddy from getting a magical key from the toad, but she could have just climbed around an old tree, gotten dirty, found an old key, and imagined the rest. When I was a kid, I know I would have imagined a dirty old key to be special/magical if I didn't know better. Del Torro deliberately made it ambiguous, so that the choice rested with the viewer, but says that he personally believes in the fantasy side more.
