Ham, if well sealed, is good in the freezer for years. The primary problem is air, which will cause freezer burn, which dried out areas on the ham. Vacuum packaging is the most effective at preventing this, followed by multiple layers of foil and plastic zip-locks with air evacuated.
According to the following webpage, cooked ham should be good for 1-2 months in the freezer.
