Well, we have some gas stations where there is an attendant, but most that I have seen are self-serve. So, both.

In the UK you pump your own, except for a few very small country garages (I believe the have older pumps with not as many safety features).

Depends on the state in the US. Most states it just depends on what station you go to, most are self-serve, but there are a few that will pump for you. I personally prefer to do it myself. I know in New Jersey, it's illegal to pump your own gas for some bizarre reason.

I only use full-service if I am going to run out of gas. It's cheaper (hahaha) at the self-service pumps. We have both here in the State of Maine, USA.
