I think it's just because of the way it's said. You might say, "I slept with Cindy" or "I slept with my co-worker", when you had sex with them. But if you just shared a bed with someone, like your brother, you'd probably say something like "My brother and I slept in the same bed", not, "I slept with my brother"

Not necessarily. I "slept with" a girl in college, but we didn't "do" anything. (In other words, in the literal sense, the answer is and can be "no".) However, in most people's vernaculars, if someone says they "slept with" someone, it usually means they had sex with them.

Because the times when beds were luxury and multiple beds extraordinary are over. That kind of "sleeping with" arrangement is just not commonplace anymore.
