About 25% of all babies are born with the cord around their necks and the vast majority have no negative effect. The care provider simply slips the cord over the baby's head as the baby is born. If the cord is tight and is preventing the baby from descending, the doctor can clamp and cut the cord to facilitate birth. In some instances, cord compression can occur, and this will cause decelerations in the baby's heartrate and will usually result in a cesarean birth.

Probably no effect at all. Unless it is so tight it cuts off circulation to the brain or circulation through the umbilical cord, but that happens rarely.
