If I blow on it and it moves it is small. If it's out of focus it's far away.

It may be far away or it may be small.Like moon it is far away,big but looks small, on the other hand microbes near to us but too small to see. Remember we studied about Angle of vision of human eyes, which was 2 degree for human eyes if I remember it correctly.It is all about the angle of vision.

This question comes up a lot when talking about constellations in space. The answer that both are factors. Take a star for example. Altering either the size or the intensity of the light at its source play a role in perception of the object. This concept can be applied to visual perception on Earth.
