Boxers usually. Briefs look stupid when you sag, especially if they are white. There are some exceptions, though. Partly it depends on how low you sag. If just a few inches or so, it doesn't matter much. If you sag so that you show your whole butt, then boxers are better looking.

A ladies thong would probably be a no-no.
