Jose A. Bautista actually was on 5 different teams in 2004 but he never appeared in a game for the Mets, so the record for actually playing for different teams is 4 by 9 different players

July 30, 2004, Bautista was traded by the Kansas City Royals to the New York Mets for Justin Huber.

July 30, 2004, Bautista was traded by the New York Mets with Matt Peterson (minors) and Ty Wigginton to the Pittsburgh Pirates for Kris Benson and Jeff Keppinger.

This is the list of Major League Baseball players that have been played for four different teams in the same season:

    1. Frank Huelsman: 1904 (Chicago White Sox, Detroit Tigers, St. Louis Browns,Washington Senators)

    2. Willis Hudlin: 1940 (Cleveland Indians, Washington Senators, St. Louis Browns, New York Giants)

    3. Paul Lehner: 1951 (Philadelphia Athletics, St. Louis Browns, Cleveland Indians, Chicago White Sox)

    4. Ted Gray: 1955 (Chicago White Sox, Cleveland Indians, New York Yankees, Baltimore Orioles)

    5. Wes Covington: 1961 (Milwaukee Braves, Chicago White Sox, Kansas City Athletics, Philadelphia Phillies)

    6. Mike Kilkenny: 1972 (Detroit Tigers, Oakland Athletics, San Diego Padres, Cleveland Indians)

    7. Dave Kingman: 1977 (New York Mets, San Diego Padres, California Angels, New York Yankees)

    8. Dave Martinez: 2000 (Tampa Bay Devil Rays, Chicago Cubs, Texas Rangers, Toronto Blue Jays)

    9. Jose A. Bautista: 2004 (Baltimore Orioles, Tampa Bay Devil Rays, Kansas City Royals, New York Mets and Pittsburgh Pirates).
