first you need to communicate with your wife. communication is mostly the reason for dissatisfaction in the bedroom.
also, romance is the key. a woman likes romance. flowers, small notes, i love you for no reason. help out around the house, etc.. dont concetrate on physical, romance the mind and the rest will follow.

- it happens. people can be born with no opening down there at all. i was born without an anus or urethra and only a vagina. This guys wife will have had a lot of reconstrictive surgery down there and possibly inside.
  
  He needs to sit down with his wife and talk to her about it, its a sensitive issue, so make sure its the right time.

talk with her about it, see if any other man made her come before... maybe she can help you with it. if not, let her go to a doctor and the doctor might be able to see if its a biological problem.
