Help answer this question below.
The rules specify that the only way the pitcher doesn't have to pitch to the first batter is for injury reason.
The manager can't simply pull the pitcher because he doesn't like what he sees from the pitcher. The pitcher must complete the first batter's 'at bat'.
as long as he does not throw a single pitch to a single batter, it does not count. in the major leagues pitchers will be pulled during warm ups if they experience shoulder problems. warm up pitches do not count towards an inning.
The manager can replace him without the pitcher throwing a pitch
Here's an article that I found
Salomon Torres left the field reluctantly after throwing his warm-up pitches in the seventh. This happened on April 20, 2003.
Also,
The Erie SeaWolves and their fans were looking for a big start from Justin Verlander Tuesday night, and through the first three innings, it looked like they were going to get it. Unfortunately, when Verlander went out to throw his warmup pitches in the fourth, he felt some stiffness in his shoulder, and Duffy Dyer removed him for precautionary reasons.
Cole Hammels (philly pitcher) pitched a bad game on Saturday. Later he said he pitched the game with a "dead arm", what does this mean?

You're reading If the starting pitcher throws warm-up pitches in the top of the third inning, does that pitcher have to pitch to one batter, or can the manager substitute the pitcher for a reliever?
