When someone steals someone else's avatar and name, because they didn't like the answers/points they were getting under another name, do you think the best route is to downrate/flag, or simply ignore them? I tend to go for ignoring them.

I'd ignore them..but if it persisted I would ask the moderators for help.

My best choice is to actually report situations like these. I happened to end up in many situations where I was made fun of, and had my name, and avatar stolen. I sent the report to the staff. After that, the activity stopped for me.

I don't downrate. Best thing to do is report them. There have been too many cases where someone has cloned an account to give the normal ABer a bad name by DRing people, causing trouble in questions, answers, and comments, and, in general, making an ass of themselves. And too many people don't pay THAT much attention to the levels, so it may take them a while to realize that it's NOT their friend. Report them... Via Feedback from THEIR profile, to a CL (see http://www.orangefrogproductions.com/ofp2m_faq_ab_policing.shtml#cl ), or to the blog. Report them as a clone, and report what they are doing.
