No a condom break is not very likly, the most likly causes for a break would either be a faulty condom, wearing a condom that is too small, od (and this is where alot of people screw up) wearing more than one condom at a time.

Not really, though certain brands have been problematic for me. I'd avoid Lifestyle brand at all costs.
