The key is misdirection. Most magicians will do a series of coin tricks involving passing it between his hands, his pockets, or wherever. Then he could 'hold out' (pretend to put it in one hand when really it doesn't move), and as the spectator is watching that hand the other hand reaches behind the spectators ear (coin is concealed in the "clean" hand) and pulls out the coin.

Coin tricks are becoming the hottest medium of close-up magic. The easiest way to pull a coin out of someone's ear is to use a French Drop. This is a basic slight-of-hand move in which you make it appear that you are passing the coin to the other hand, but instead you really keep it in the first hand. There are so many ways to make things disappear that it would be impossible for me to name them all.
