It will come down to genetics eventually but it is mainly due to the incorrect amount of testosterone introduced during development within the womb. We all start off female and at the right time, as determined by our genes, testosterone is pumped in to turn us male or keep us female. If too much testosterone is pumped in on an XX embyo then the girl will have masuline qualities, even a larger clitoris (which actually looks like a very small penis). If too little testosterone is pumped in on an XY embryo then the boy will have feminine qualities, even a very small penis (making it difficult to distinguish from a clitoris).

the term androgynous refers to a superficial apperance having nothing to do with genetics. maybe you meant intersexed(also known as hermaphrodite)? here is an excellent source to find out all sorts of information about intersexed persons http://www.isna.org/
