Movies really don't frighten me, so the only times I was ever frightened was when I was really young. There was 2 that got me way back then. "The Haunting", (original black & white version, must be watched late and alone), and "The House of Dark Shadows", (I was in 4th grade, and dreamed all night about Caroline calling that little boy, for a bite). One other movie really made me think, and it's been mentioned a few times already ... "The Exorcist".

The Ring - man, I hate that stacato walk (spelling?). The Exorcism of Emily Rose was pretty scary, too.

At the time, about 1980, I was terrified by Salem's Lot. Not watched it since but had horrible nightmares because of it

The most scared I ever was was when I was a kid and watched The Wizard Of Oz. One that comes to mind that nobody's answered yet is Cape Fear, with Robert DeNiro as an ex-con stalking a family... really scary.

The Hills Have Eyes scared the pants off me. And it was also a very gory movie. I had a hard time sitting through the movie.

1922 Noseratu First of all Max Shreck plays the most disturbing, scariest vampire eve on the screen. For close to 100 years the appearance of Na Max Schreck with his gaunt face, crooked nose, horrific expression can still send chills. Nosferatu was based on Bram Stoker's Dracula. This makes any other Dracula seem like a Disney fairytale BTW Max Schreck is German for ultimate terror I believe Shrek name was inspired by Schreck.
